TERMS and CONDITIONS I hereby enroll my child in the school instrumental music program. Learning an instrument is a commitment in which I will support and encourage my child to remember his/her instrument each week and establish a regular practice routine at home. I understand there are no refunds after the third week of classes. A minimum number of students is required to hold a class and cancelled classes due to low enrollment will be issued full refunds. The Instrument Place will also refund any orders cancelled due to cancelled classes. I understand that regular attendance is very important and that three or more unexcused absences may result in my child being dropped from the program. I also hereby recognize the importance of having a good, quality, working instrument and I understand that my child must have an instrument brand name listed on the approved brands list in order to participate. Inferior quality instruments are not acceptable for use in the music program. I also hereby grant to Children’s Music Workshop, its sponsors, organizers, and/or assigns (“CMW”) the irrevocable and unrestricted right to use and publish photographs (including video photography and performance) of my child, or in which my child may be included, for publication, electronic reproductions (web sites) and/or promotional materials or any other purpose and in any manner or medium. In addition, I grant my permission to alter the same without restriction; and to copyright the same. I hereby release the photographer and CMW from all claims and liability relating to said photographs.
