Instrument choices vary from school to school.  If you are unsure of the instruments offered at your school, please revisit the school's music web page.

Check "Beginner" if your child has no prior experience on the selected instrument. Check "Intermediate" if your child has six months to one year of experience. Check "Orchestra" if your child has more than one year of experience. Please provide us with additional information in the Comment Box.

Check "Will order online" if you plan to place an order with our recommended instrument supplier The Instrument Place. All instruments from The Instrument Place are program approved. Check "Owns" if you already have a program approved instrument from another vendor. Check "Other" if something else applies and explain in the Comment Box. Check "Recorder" if enrolled in a recorder class. (Recorders are provided free with enrollment in the recorder class.)

Check "In the Mail" to indicate that you understand your paperwork must be received by Children's Music Workshop within 72 hours of this online registration to hold your child's seat in the class. Check "Other" if something else applies and explain in the Comment Box. Check "UCLA Lab" if you attend the UCLA Lab School. No paperwork is required for UCLA Lab students.

TERMS and CONDITIONS
I hereby enroll my child in the school instrumental music program. Learning an instrument is a commitment in which I will support and encourage my child to remember his/her instrument each week and establish a regular practice routine at home.

I understand there are no refunds after the third week of classes. A minimum number of students is required to hold a class and cancelled classes due to low enrollment will be issued full refunds. The Instrument Place will also refund any orders cancelled due to cancelled classes.

I understand that regular attendance is very important and that three or more unexcused absences may result in my child being dropped from the program.

I also hereby recognize the importance of having a good, quality, working instrument and I understand that my child must have an instrument brand name listed on the approved brands list in order to participate. Inferior quality instruments are not acceptable for use in the music program.

I also hereby grant to Children’s Music Workshop, its sponsors, organizers, and/or assigns (“CMW”) the irrevocable and unrestricted right to use and publish photographs (including video photography and performance) of my child, or in which my child may be included, for publication, electronic reproductions (web sites) and/or promotional materials or any other purpose and in any manner or medium. In addition, I grant my permission to alter the same without restriction; and to copyright the same. I hereby release the photographer and CMW from all claims and liability relating to said photographs.

When all required fields are completed, the form will be accepted and you will be directed to a "CONGRATULATIONS" page that includes your completed form. You will also receive a copy of the completed form via an auto- response email.

Please print a copy of the "CONGRATULATIONS" page or the auto-response email and mail it along with your check to the P.O. Box as directed. (Please be sure your email address is entered correctly.)
