MUSIC APPLICATION FORM
This form is a general form for all Children's Music Workshop schools.
Payment is not done online - just registration. Please
refer to your school's orchestra program webpage for
mail-in payment information grade/instrument restrictions. For
your convenience, you will be taken to this page after you click
on the "Submit" button at the bottom.
Please note that
class size is limited and acceptance is based a on first come, first
served basis.
After a
class is full, no more applications can be accepted.

TERMS and CONDITIONS
I
hereby enroll my child in the school instrumental music program.
Learning a instrument is a commitment in which I will support and
encourage my child to remember his/her instrument each week and establish
a regular practice routine at home. I understand there are no refunds
after the third week of classes. I understand that regular attendance
is very important ant that three or more unexcused absences
may result in my child being dropped from the program.
I also hereby
grant to Children’s
Music Workshop, its sponsors, organizers, and/or assigns (“CMW”)
the irrevocable and unrestricted right to use and publish photographs
(including video photography and performance) of my child, or in which
my child may be included, for publication, electronic reproductions
(web sites) and/or promotional materials or any other purpose and in
any manner or medium. In addition, I grant my permission to alter the
same without restriction; and to copyright the same. I hereby release
the photographer and CMW from all claims and liability relating to said photographs.
